To sign up for any of our leagues, tournaments, clinics or camps, please complete the individual registration form which includes the Waiver and Release Form.

The Waiver and Release Form is now a part of the Individual registration, which must be filled out.

TEAM REGISTRATION FORM - PLEASE REMEMBER THAT ALL PLAYERS ON YOUR ROSTER MUST PROVIDE A INDIVIDUAL WAIVER/REGISTRATION FORM . WAIVER/REGISTRATION FORMS ARE GOOD FOR A ONE YEAR PERIOD FROM SEPT 1ST TO AUGUST 31ST
